Campaign Supply Checklist (.doc)  
Employee Campaign Coordinators should fax this form to the United Way office at 262-338-3465 in order to schedule pick up or delivery of supplies for their workplace campaigns. Please allow at least one week prior to delivery or pick-up for supply requests.

Campaign Report Envelope Instructions (.pdf)
